My best guess is still the tensioner even though a new one was installed. Parts do fail, and perhaps the spring on this one broke. An other guess is the tensioner was not torqued properly. As you know, you do torque it lightly to do the setting. Perhaps it wasn't torqued the rest of the way? Please keep us posted!
