I liked this thread for the great pictures and write-up you posted, so figured I'd keep it going for other's benefit.

I think I figured it out though... the current EQ plug has the left-in --> left-out connected and right-in --> right-out connected. It looks like you did the same to connect them on the 1/8" jack correct? (I was confused why you had 5 wires going to 3 contacts on the 1/8" jack).
