One question...does this happen on cold start, when the engine is warmed up, or both? Does it seem different when cold than hot?
I second checking the intake manifold gasket. The symptoms could all be related or they could be separate issues. Without seeing the car in front of me, it's difficult.
To your question about vacuum...it's all about rubber...and our Volvos are known well to outlive the rubber that seals everything up.dry, cracked, and leaking vacuum hoses cause a variety of problems, ranging from running poorly to your dash vents not working right to the cruise control not holding. Solid vacuum is very important and, at this age, we're well beyond the normal lifespan of natural rubber. It can easily be tested to isolate where the problem is. If you have a few hours, it's wise to just replace all of the hoses. I did all of mine after my dash vents stopped working and it made a world of difference in the overall driving experience. I bought a roll of vacuum hose and made my own, using the rigid, heat resistant hoses where applicable
As for the cruise control...In addition to the hoses going into it, the cruise servo unit is known to get little holes in the rubber bellow that will cause the cruise to lose vacuum and not hold speed. It gets progressively worse. Another member, Philip, suggested using a tire patch on the bellow to seal a hole and that worked for him. Otherwise, the servo itself with a new bellow is not that expensive to just replace with a new one. It takes all of 90 seconds to replace, start to finish. Vacuum canisters seem to fail less in my experience but it has been known to occur.
I'm assuming you have no Check-Engine Lights. If you did, similar symptoms manifest themselves with faulty Air Mass Meters, but a faulty AMM would most certainly trip the Check Engine Light, often with a Lambda code.
