Timing cover.... Gotta be a timing gear headed south!
They all do it eventually. The noise is scary and sometimes they will slip timing and then
come right again and run OK for a while.
Better get a new timing gear!
This'd be a great time to put in the later style seal too. I have a few updated timing covers.
