Everyone pays something called "National Insurance", automatically deducted from wages / salary. The government then pay GPs a salary (they're employed by a "Health Department"). They can take private patients as well if they wish. When you become ill and maybe need hospital treatment, it's all free at the point of use. No-one is afraid of getting ill, no-one is too poor to receive medical care, no-one is at the mercy of an insurance company. Simply, isn't it? Well it's worked here since 1949. Regards.
