Surging S80 1999

My 1999 S80 T6 surges at idle and sometimes at highway speed. Had throttle body replaced including software upgrade to ecm. Ok for a while then reocurred. Had TB cleaned--ok for a while. Same problem again. Can this be as simple as a dirty tb and if so, what is the procedure for owner maintenance? thanks.
