The rear O2 sensor for cyls 1,2,3 needs to be replaced. Your mpg is bad because of unmetered air entering the manifold, which could be a simple vacuum leak or a poor responding MAF, along with a bum O2 sensor.

That is just a start. If there is an exhaust leak, that will mess up your )2 sensor readings and contribute to the problem.
