Just the lenses, about $100 each. They're Volvo OEM. To change over, remove the headlight housing, then remove the retaining clips holding on the old lens. Installation is the reverse of this procedure. Should take about 20mins total. All you need is a 10mm socket with extension, and a screwdriver.

Can't answer your question about the D2R or D2S as I don't know what the shield is for, nor am I thoroughly conversant in HID. In either case, they're designed for E-code installation. The light pattern (including the cutoff) is created by the lens & reflector, not the bulb.

I've read rave reviews of Euro Volvos with HID's and I plan to make the change when prices come down to earth.
