The top mount is the first suspect, and get a poly bushing from IPD. They last forever and are easier to install. Some board members feel they are too harsh and transmit vibration in to the car body, I don't notice mine.
Flush the tranny to eliminate it from the surging problem. The throttle body probably needs cleaning and the vacuum elbows need to be checked for cracks.
