It is hard to determine what is wrong with the car from our viewpoint. The sudden high revs indicate that a hose came loose allowing air into the manifold, or an injector broke and poured fuel into the manifold. The very high revs for a diesel could have destroyed the turbo, which then allowed oil to enter the exhaust manifold and created the white smoke.

That one piston is dead could have come from a broken injector. It is common for diesel injectors to become partially clogged depending on the cleanliness of the fuel. I suspect the injectors could be cleaned rather than replaced, most truck firms will know where to get injectors cleaned.
