What is code 21? I know 67 is normal; that (IIRC) is the neutral drive switch or clutch switch, which is not hooked up. 21 could be ECT though. It definitely sounds related to ECT, the way it is starting.
Does it crank fast enough when hot? The only time that car ever gave me starting trouble was when its starter was dying; when it would get hot it would not crank fast enough. However, that has been replaced, so it should not be the problem.
An ECT costs about $20 and takes two minutes to swap out. Give that a try. I also sent you a sheet in the folder of stuff that tells you what all the sensors should be reading under certain conditions (cold idle, warm idle, cruise, etc), and if you have a volt/ohm meter, you can back-probe the ECT plug and see what it is saying with the engine warm. If it is out of spec according to the sheet, you have found your problem.
