If the problem happens coming off idle - either accelerating from a stop, or accelerating after coasting on the highway - I'd put your IAC valve high on the list of suspects. Cleaning my IAC cured this exact symptom. Have you cleaned your throttle body and IAC valve, and checked your throttle position switch function?
