What's the difference in operation between the crankcase breather, which goes from teh side of the block to the air cleaner, and the PCV hose, which goes from the oil filler cap to the intake manifold?

Are pressures inside the valve cover different from pressure in the crankcase?
Is there more oil vapor in either one?

Would there be any point in combining them?
