I had a similar problem that took me forever to track down, it turned out to be a leaking intake manifold gasket.

I would also have a real hard look at your patched air tube bellows thing, and make sure it really is leak-free.

Is it a regular miss on one cylinder? What happens if you disconnect the fuel injector wire connectors one at a time?
