My thought is maybe the nut holding the bearins is loose. when the tire is jacked of the ground, grab the top and botton of the tire and see if there is movement. If so, either your balljionts are shot or simply you may have the nut holding your bearings in loosened. take off the cap in the center of the tire (big pliers) and see if it's loose. I don't know what the torque is. If I remember correctly, there is a cotter pin going threw the crown nut.
