You don't need an adapter as Lukeball mentions (at least, I didn't). My RX7 already had two vertical holes with diameters and spacing very close to that of the mounting bosses on the firewall.
1) I'm guessing you have a remote (not firewall) mounted booster, like most 122's? My car has a Lockheed remote mounted booster. When I redid the brakes I bypassed it, but only because I wanted to remove potential sources of confusion while I troublehsot and tested the new MC setup. The booster has since been rebuilt and will be reinstalled. With this type of setup, you will only be boosting ONE circuit (typically the front) since the booster has only one inlet and is in place AFTER the MC.
2) I believe it is the same
3) I think that is what I ended up doing. Removed the clip that retains the pushrod on the volvo MC, and used it on the RX7 MC.
