How does a FPR usually fail?

So I swapped in a new FPR a week ago, and I think it may have already failed.

When I swapped in the FPR the car was running great, and I was getting proper switching on the o2 sensor(which hadn't been switching before).

Now the car is back to running poorly. Low power, hesitation. I think it is running rich again. Also the o2 sensor is back to a non-switching state(more or less stable .8v).

When I pull the vacuum hose off the FPR, there is no change in idle rpms. I also smell fuel at the FPR.

So what gives? How could my FPR have failed in 1 week? I know that my fuel filter was a little clogged, but I just replaced that yesterday.
