I may have found the answer in the FAQ 200 1990

A couple of months ago my 240 started running poorly and my miles per gallon dropped. I replaced the Fuel pressure regulator and O2 Sensor with no change. I brought it to the area volvo "expert" who changed the plugs and wires and made a few adjustments. It runs a bit better but still not great. MPG is still down. He said "It is strange, the codes say it is running lean, but it is actually running rich."

Now I think finally found the right spot in the FAQ under electrical connections. It looks like the mechanic could have quoted the FAQ. I will be checking (maybe just replace) the ground connections for the injectors tonight.

Wish me luck! This has been a pain.

Good luck! Consider your engine coolant temp sensor. Mine was bad and the symptoms were similar (running rich, black smoke from the tailpipe, very bad MPG, etc.).
