How long should a Catalytic Converter Last? 200 1991

I replaced mine last year and I again failed the inspection. I am sure that it is an indicator of other problems but the inspection guy seems to believe that older cars need them replaced every year or two.

You are correct. It is an indication of other problems.

My '91 has 285k miles on the original cat and and recently passed California emission inspection easily.

Is the inspection guy selling them? He is incorrect.

They can last forever if the car is maintained properly. Long time running rich will soot it up, it is then useless.
The definition of a catalyst is that it changes or speeds up a chemical reaction without undergoing a change itself.

Well, the guy might be right, if the "old cars" are burning lots of oil and running super rich - those conditions will fubar a cat before long.

Start with the usual places - fuel pressure regulator, make sure ignition's in good shape, then check the oxygen sensor. Any codes set?

A real long shot (so don't go here first) is an odd condition where a cracked exhaust downpipe ended up sucking in air upstream of the oxygen sensor, tricking it into seeing a lean condition that didn't really exist. The ECU over-compensated by creating a rich mixture.
