PCV system service - 2000 V40 1.9t S40-V40

The Volvo owner's manual says: "PCV SYSTEM -- The orifice nipple in the intake manifold and the filter at the end of the PCV hose in the air cleaner should be inspected at 60,000 miles and thereafter, at 30,000 mile intervals."

Q1 - Is the "nipple in the intake manifold" the tube that goes inbetween the spark plugs? If so, how do you remove that hose clamp?

Q2 - Where is the "filter at the end of the PCV hose in the air cleaner"? Is this just the actual air filter they are referring to?
