The common failure or separation is the upper strut mount which in your case cost you about 30 dollars. This strut mount is made of rubber and is seated on top of the compressed spring. The metal mount (per your description) is actually a bearing on a metal plate. The metal plate has 3 screws pointing upward at 3 different corners. This metal mount seats on top of the rubber upper strut mount. Then a smaller metal center disc and the big strut nut are on top of the metal mount.
