cluth fork 200 1984

while comeing to a stoplight, i pushed in my cluth pedal and to my dismay it stayed on the floor and didn't realease the clutch. after some stressful double clutching in traffic i was finally able to get it home and found the pedal and cable to be sound. i found the clutch fork to be very loose and and the pins that hold it to the shaft to be unattached, if it weren't for the shaft i think the fork would pull right out. Is this a common failure? I have not pulled the tranny yet but would like to know what could cause this and if i should replace additional components??? the cluch has always held strong and never given any problem until now. Any input would be appreciated.
