wheel bearings 200 1987

How do you know your wheel bearings are bad (front)? I have a rattling steering wheel, a whine coming from the wheels (might be the new tires, though), and the other day a grating sound when I stepped on the brakes in the parking lot. 2 days before that I had the car in the garage to check on the rattle coming through the steering wheel and they tightened the wheel bearings which were loose - everything else was okay w/front suspension. Is there something going on? How do wheel bearings come loose? What happens when they fail? Thanks.
