91 240 poor idle when warm 200

I have a 91 240 with auto trans. 135K miles. Car runs fine except when idling after the engine being warm. After driving normally for a half hour or so, if I stop for a bit and the car is still warm, when I start again, the engine idles low and the car tends to stall when shifting into gear (unless I give it a bit of gas). Once in gear and driving, it is fine. I really don't like feeding in the gas while shifting the automatic to keep it from stalling, any ideas?
