88 740 Wagon exhaust smell 700

When I start the car (330k, and in great shape) I get an exhaust smell in the cabin. It seems to lessen or disappear after a number of warm starts when doing errands around town.

Any ideas of what to fix? I have a carport to work in but no lift other than a pair of ramps.
