WRIST PIN NOISE 200 1980

I have a 1980 B21A Carb with 427,000 Km - runs well no oil consumption. Has the usuall piston slap on cold starts for ten minutes but my mechanic said there was wrist pin noise as well - expensive to fix.
So I need the bottom line - is this 'noise' anything to worry about?
Will the engine self destruct? Or will it just continue to run noisy and go for another 200K up to say 700,000KM?
