I was getting a heck of a rattle from the exhaust system of my 1990 740 16v, and had no idea what was causing it ... it was a very loud clanging rattle, usually only on startup or when the engine RPMs were really low, like at idle. Also, it would often set off a good rattling spell when I'd take off from stoplights, etc.
Got up under the car with a rubber mallet and started whacking on various exhaust parts, and tracing down the rattling noise. Discovered that the manifold-to-downpipe connection had gotten a bit loose (held together with 3 flanged nuts on the underside, with a gasket between the parts). The previous owner had apparently lost a couple of the Volvo flanged nuts and had gone back on with regular nuts without the flanges, which loosened over time.
I picked up a couple of used nuts from our local Volvo junkyard, a new gasket, tightened everything up really snug, and no more rattles! You'll need a 15mm socket, a 6" extension, and a U-joint to do this repair, from underneath the car. The nuts are impossible to get a wrench on from the engine compartment side, and you'll need to snake in with a U-joint from underneath to reach the nut closest to the firewall.
Lubed all the nuts with silver Permatex anti-seize grease before reinstalling. 500 miles later, and the exhaust system is quiet as can be.
