Bad injector code? 200 1990

I have a 1990 240 wagon with an LH 2.4 fuel injection system.

The other day I started my car (warm start) and the engine hesitated and shook badly. I gunned the throttle and the idle smoothed out but I still experienced a loss of power as I started out in first from the next few stops. Then the check engine light came on. I got a 2 3 1 code (compenstating for a too rich / too lean mixture) and a 1 1 3 code (faulty injector(s)). The injectors check out electrically and when I pulled them, they had no carbon deposits on them.

I also ran through a tank of gas with that fuel injector cleaner, but I still get the same code.

When I reset the ECU memory and then start the car, I immediately get the check engine light and the same two codes.

The car runs great, and my gas mileage is normal.

Is it possible that my O2 sensor is going bad? I know there's a code for this (a code I'm not getting) but the injectors seem to be okay. I've also checked the vacuum lines for leaks but I can't find any.

Can anyone offer any insight? Thank you.

    The injector code on LH2.4 is not actually a direct injector diagnostic indicating any thing electrically wrong (It is on M1.8, M4.3, M4.4, etc).

    It is a code thatsets when the LH ECU adjusts fuel mixture over a certain percentage without seeing a change in Oxygen sensor voltage output.

    The assumption is that a clogged injector would not be delivering a marked increase in fule despite an increase in opening time as controlled by ECU.

    Check the Oxygen sensor thorughly whenever mixture codes show up as the O2 sensor is the source of any info the ECU uses to determine that air/fuel mixture is not correct.
