I really for the life of me cannot locate any middle rear seat belt in my 1991 745. Am I truly missing the obvious here?

Well, maybe the PO has hidden it underneath the lift-up seat bottom. Have a look there. If it's not there, maybe the PO unbolted it and removed it entirely.

Yes, I certainly did check under the seats and all around. I cannot find the shoulder belt is the problem. Where would it normally be bolted in from? Or, really embarassing, is it somehow the same belt that is used for the third seat in the wagon back...?

Umm, the shoulder belt? The middle seat belts didn't have shoulder straps until later models, just a single lap belt. The third seat seat belts are entirely different.
