2002 S60 Headlight Assembly wet S60

90k miles. The passenger headlight assembly looks like it has condesation in it, but doesn't look like it's inside the light housings themselves.
- is there a layer of glass or plastic between the outside glass and the light units themselves?
- what would cause it to get condensation? Driver side is dry and clear.
