1999 Volvo S80 T6 tranny - shudder and slam! S80

Ok, I admit I'm one of the idiots that has kept this sled for too long. After having the tranny rebuilt at 125K, it just turned 150K and it is slamming into 2nd gear and sometimes will wind up to 3000 rpm before shifting. This usually happens after I exit the freeway. At times it will also stay in 3rd gear - makes it real interesting in the slow lane of the freeway.
Any hints before I get rid of it?
