2004 V70 Window drops off track, what goes wrong on these? V70-XC70 2004

I have an issue with my 04 V70, the driver side window sometimes drops while operating. Something is out of whack, and I am afraid it will inevitably crash down in the middle of a downpour when I open the window at the toll booth. I did take out the window switch to see what I could see, and I found a round black plastic disc about the size of a silver dollar, with a screw in the middle of it affixed to a post. This part was just floating freely inside the door. I have no idea as to what it is, but I have to guess it has something to do with the 'off the track' issue. I found a description on how to remove the panel at:

But before I dig into the door, does anyone know what the failure points are, and what I might need to have on hand to address the issue?

    You will need to remove the door panel and reattach the disk/washer to the window guide. The guide being out of place is your problem.

      OK- I took off the door panel:
      1. Popped off triangle plastic piece right above the door panel
      2. Popped out the plastic rivets by placing a very large flat screwdriver under the door panel at each rivet, and they popped right out with the centers still inside(I know some folks said to drive the center pins through, but I didnt find that I needed to)
      3.lifted the window switch out and disconnected the two interfaces.
      4.unscrewed 2 torx screws inside the pull handle(after popping off the cover)
      5.lifted the panel up and over the lock button and disconnected the speaker wire.

      Now for the hard part of my job. I simply can not figure out where that little black plastic disk with the screw in the post goes???? I looked and looked and it is far from obvious where that part fell off from?? I ran the window up and down and watched, but still I could not see where it goes? Help....

        As you can tell, you are in unchartered territory with your 'new' car. On the 850's the rear window guide used to break off with the same result you are getting. Finding a small screw hole is going to be a good sluething exercise. I would start by gently pulling on window guides and see if any are loose.

          I think I will have to stop by the dealer and ask to see the exploded diagram to figure it out, or perhaps I can find it online. The parts that I assume you are referring to as guides(blue plastic pieces that slide in the channels) are both there. One of them had the ball popped out of its socket, but I put it back and replaced the retaining clip(which was not on there), but that didnt solve the problem of the window not going up and down properly.

            The channels themselves, not the plastic inserts. Getting a look at a diagram would make a lot of sense, my dealer parts guys do that for me also. Sometimes they even give me a printout to take home - bonus.
