If inserting the spacer made the insert too high and looking in there
you see a taper you have a spacer in there. If there wasn't a spacer you
would see the retaining nut on the ball joint. The only cars missing the
spacer are turbos. They have a longer large shaft gas shock from Boge
so no spacer needed.

The spacer under the top of the insert slid onto the top tapered part and
took up room to help it fit tightly. The KYB nut was the only one I could
use. The volvo nut wouldn't fit to hold the strut in there properly. The
strut tube is the same except for the bottom spacer on turbos so I don't know why the KYB spacers are so big. When I would tighten the KYB top nut there would be a few threads left. That was not a problem. From what you
described you may have fixed it when you changed to the KYB retainer
nut. When I tried the Volvo nut on KYB inserts it would not tighten
down enough to secure the insert in the tube. I think the volvo nut only
works properly with Boge automatic, Turbo gas, and Pro Gas shock inserts.

I also use a pipe wrench for the insert retainer nut. Mine is only a 12"
version so I would think you are getting the nut tight enough.
