I had the same problem a few years ago on an 82' 240 turbo w/ automatic. Since the O.D. does not effect the gearing until you are in 3rd (as far as gearing the car down when the O.D. is off), I would say that this problem is not related to an overdrive. You can not get out of first until the car warms up, so this is a transmission (internal) problem. -I would have to drive for a few miles when the car was cold until I could finally go faster than 20 MPH when it would start shifting. I flushed the entire system, changed the transmission filter, added some magnets to the inside of the pan and added a recommended transmission smooth-shifting additive. None of this worked because the transmission internal parts were too worn. I ended up buying a used transmission for $400 from a wrecking yard, which is still (7 years later) shifting smoothly.
