I had my LCD backlite bulb burned out also at about 20k miles. Dealer replaced the bulb. The charge of the bulb was about $2.00. Dealer changed the bulb when I was in for 20k maintaince. Dealer told me that DO NOT turn the instrument light intensity all the way up. So many owner did that and had the bulb burned out. Now, I set the instrument light to "half" way. So far so good, my car has 44k miles up today.

I don't think that you have to remove the whole dashboard. But this is just my guess only.

For the driver side visor light, I had the same problem as you experienced since day 1 I had my car. Dealer fixed it 2 times, until recently, they replaced the whole driver side visor due to a small crack in the vinyl. I haven't seen the problem with the new visor so far. I hope that it is not going to happen to this new visor.

There is a light in the armrest. When you open up the armrest, you will see a white plastic located in the back of the storage area. That bulb is used to light up the armrest storage area and the ash-tray behind the armrest.
